Together sought to remove friction from patients’ medication management journeys by automating prescription tracking, clinical reminder scheduling, and refill logic. The goal was to create a platform where users could effortlessly receive timely reminders, refill prompts, and test notifications without worrying about complex pharmacy processes or missing doses. By providing a single access point, Together empowered users to manage prescriptions and health records seamlessly, ultimately improving medication adherence. The platform’s value extended to providers and pharmacies as well, reducing manual follow-ups and freeing clinical staff for more meaningful patient engagement. Challenges

Designing robust logic to parse and store RX numbers, pharmacy origins, prescription dates, and refill triggers from a wide range of prescription layouts proved demanding. Variability in data standards, provider fields, and user-uploaded images introduced edge cases requiring extensive validation and AI-aided correction. Integrating with major external pharmacies, especially those with complex and evolving APIs, meant adapting to changing requirements while never compromising on privacy. Finally, generating useful summaries from clinical documents and providing automated reminders demanded smooth backend–frontend orchestration to prevent errors that might leave patients non-compliant or delayed.

 

Process

The project began with extensive patient journey mapping and pharmacy workflow analysis to identify bottlenecks in repeat prescriptions and missed appointments. Backend development focused on building routines for parsing images, detecting RX numbers, matching pharmacy identifiers, and aligning with refill cycles. The engineering team developed workflow engines to synchronize with CVS and Walgreens APIs, facilitating automated refill orders and notifications. Clinical report management modules extracted and structured key test results. On the user front, the design team built dashboards, reminder logic, and notification settings for proactive engagement. Security testing and HIPAA compliance remained at the forefront, guided by ongoing feedback from pharmacy partners and early adopters. This end-to-end process enabled rapid iteration and continuous enhancement, always centered on patient safety and seamless pharmacy operations.
